Immigration Physicals

Dr. Irfan Syed is a certified civil surgeon designated by the United States Citizenship and Immigration Services(USCIS). 

The examination includes the following:

·         Physical exam by our civil surgeon.
·         TB testing (age 2 years and up)
·         Blood tests for Syphilis & Urine test for Gonorrhea (age 15 years and up)
·        
Discussing and recommending vaccination requirements for the completion of your I-693 form.
·        
Sealing the completed I-693 form in secure envelope and providing full copy records for patient


 



Examination fee is $399 (includes: Exam, required bloodwork and urine test)

 



Vaccination Requirements:

 

By law, for the immigration physical exam the required vaccines include:
·         Mumps, measles,rubella (MMR)
·         Tetanus and diphtheria toxoids
·         Hepatitis B
·         Varicella
·          Influenza - only if in season (Sept 1st thru April 1st)


If you have never received or are unable to prove that you received certain vaccines, we will refer you the immunization clinic to receive the necessary vaccines or we can run laboratory testing for immunity at an additional cost per test. 

Tetanus Diphtheria titer – additional $45
MMR titer – additional $45
Varicella titer – additional $45
Hepatitis B – additional $45

If your TB Skin test is positive you will be required to get a chest x-ray. The price of the physical does not include the chest X-ray. You may be able to use your private health insurance to pay for the x-ray. 

 
When you are coming for your immigration exam please remember to bring the following items with you.

        Passport or other gov issued photo ID (Must be current)
        Medical Records if available (translated to English if necessary)
        Proof of your vaccinations if available (Translated to English if necessary)

Dr. Syed is not responsible for determining whether an applicant is actually available for adjustment in status. That determination is made by a USCIS officer after reviewing all relevant records, including the report of the medical examination.
